چکیده

Method We analyzed 20 consecutive patients (14 male; age 64 ± 12 years) with first acute inferior MI due to proximal right coronary lesion. All patients were assessed with cardiac MRI after primary percutaneous coronary intervention and followed up for a median (25, 75 percentiles) of 6 (5, 6) months. We assessed the association of RV function with length of cardiac care unit (CCU) treatment as an index of short-term prognosis by using multiple regression analysis, and used Kaplan-Meier analysis to demonstrate the association of RV dysfunction with occurrence of heart failure (HF) after discharge as an index of long-term prognosis. Results RV involvement was diagnosed with delayed enhancement MRI in 11 patients (55%). Patients with RV involvement had lower RV ejection fraction (EF) (32.4 ± 8.6 % vs 43.2 ± 7.7 %; p = 0.009) and longer duration of CCU treatment (5.3 ± 2.3 days vs 3.5 ± 1.9 days; p = 0.038) than those without RV involvement, although there were no significant differences regarding clinical, angiographic and the other MRI characteristics such as left ventricular parameters. By multiple regression analysis, RVEF was an independent predictor of length of CCU treatment (standardized partial regression coefficient -0.799, partial regression coefficient -0.127; 95% confidence interval -0.222 to -0.031; p = 0.017). KaplanMeier analysis demonstrated that RVEF <30% was associated with increased occurrence of HF in chronic phase (p = 0.003).

Prediction of long-term cardiac events by 123I-MIBG imaging after acute myocardial infarction and reperfusion therapy

Objective(s): In heart failure, the heart-to-mediastinum (H/M) ratio of the delayed image and washout rate (WR) are well-known as a powerful cardiac event predictors. H/M ratio quantifies the accumulation rate of MIBG in the myocardium and WR quantifies reduction of meta-iodobenzylguanidine (MIBG) accumulation in the heart from the early planar image to the delayed pla...

Comprehensive right heart systolic function assessment using cardiac magnetic resonance imaging after inferior ST elevation myocardial infarction

Background Acute Inferior ST Elevation Myocardial Infarction (Ac-Inf-STEMI) and associated posterior and right ventricular (RV) infarction has been extensively studied with electrophysiology and echocardiography. A recent CMR study showed that poor RV function was associated with poor long term survival post myocardial infarction. However, limited CMR data exists especially on short term clinic...

Right ventricular dysfunction assessed by cardiovascular magnetic resonance imaging predicts poor prognosis late after myocardial infarction.

OBJECTIVES We sought to determine whether right ventricular (RV) function late after myocardial infarction (MI) impacts long-term prognosis. BACKGROUND Right ventricular failure predicts early mortality in patients with acute MI. The prognostic impact of RV function late after MI is not well defined. Accordingly, we determined whether RV dysfunction late after MI influences survival beyond tr...

Assessment of Myocardial Infarction by Cardiac Magnetic Resonance Imaging and Long-Term Mortality

BACKGROUND Cardiac magnetic resonance imaging provides detailed anatomical information on infarction. However, few studies have investigated the association of these data with mortality after acute myocardial infarction. OBJECTIVE To study the association between data regarding infarct size and anatomy, as obtained from cardiac magnetic resonance imaging after acute myocardial infarction, and...

The Correlation between Left and Right Ventricular Ejection Fractions in Patients with Ischemic Heart Disease, Documented by Cardiac Magnetic Resonance Imaging

Introduction: The correlation between right and left ventricular ejection fractions (RVEF and LVEF, respectively) has been studied in only a small number of patients with a marked decrease in RVEF and LVEF. The aim of the present study was to compare LVEF and RVEF in patients with ischemic heart disease. RVEF and LVEF were measured by Cardiovascular Magnetic Resonance (CMR) imaging. Materials a...
